Insurances Accepted

We are in-network providers for the following insurance carriers:

1)  We will send out a pretreatment estimate for patients for major dental services, including crowns, bridges, partials, dentures, and scaling and root planing.  Pretreatment estimates are not a guarantee of payment. 
2)  Patients are ultimately responsible to pay for their dental services, if their insurance carrier does not pay.
3)  Most insurance carriers mandate the patient pay a copay for dental services. 
4)  The patient is responsible to pay the copay and deductible at the time of service.
